Authorization of the certificate by a Board of Education in Poland This applies to certificates from countries outside the EU, OECD, EFTA, EOG, or those with which Poland has education agreements. This document is mandatory
Apostille or legalization of the certificate together with grades list The apostille/legalization must be done in the issuing country. The university may request this during the recruitment process

Personal Questionnaire Generated in the system, printed out, and signed
Request for Admission Generated in the system, printed out, and signed
Passport Data Generated in the system, printed out, and signed
Please note that an educational gap can not be longer thatn 5 years.
Documents Required After Arrival at CUT:
Sworn translation of your secondary education certificate into Polish The translation must be either sworn or certified by a Polish consulate and include your grades list. (The university may require this document)
Sworn translation of the eligibility statement into Polish A translation (sworn or certified by a Polish consulate) confirming that your certificate allows you to apply to a university in the issuing country. (The university may require this document)
Health Insurance A sickness and accident insurance policy for your period of study in Poland, or a European Health Insurance Card, or an agreement with the National Health Fund
Up-to-date Photo One recent photo (size: 35x45 mm). Write your name on the back of both the photo
SECOND CYCLE (Master Degree)
Documents Required During the Recruitment Process:

A first cycle studies diploma with transcript
Apostille or legalization of the diploma together with transcript The apostille/legalization must be done in the issuing country. The university may request this during the recruitment process
Confirmation of tuition fee payment

Request for Admission Generated in the system, printed out, and signed
Passport Data Generated in the system, printed out, and signed
Please note that an educational gap can not be longer thatn 5 years. CGPA 60% is the minimum, unless kwalifikator.nawa.gov.pl indicates otherwise.
